Minnesota Life - Life Insurance 2008

Basic Life Insurance|

Your life insurance value is equal to one times (1X) your annual salary, rounded up to the next higher thousand. The plan also pays one times your annual salary for accidental death, 50% for accidental loss of sight, and 50% for accidental loss of an arm or a leg. This coverage is paid for entirely by Lee County. Employee's in pay grades 21 and above receive 2X annual salary up to a max of $350,000.00.

Optional Life Insurance
Optional Life Insurance is a voluntary, payroll-deducted term life insurance program designed for employees who desire to purchase additional life insurance coverage for themselves, their spouse, or their children. Employees may apply for coverage in $1,000 increments, up to 10 times their annual salary, to a maximum of $500,000 ($25,000 minimum). Employees may apply for spouse coverage $1,000 increments, to a maximum of $250,000 (also a $25,000 minimum). Child coverage is also available in $5,000 increments up to a maximum of 50% of your coverage or $25,000 whichever is less. This coverage, if elected, is paid for entirely by the employee.

Long-Term Disability Insurance

This plan replaces 60% of your pre-disability income if you experience an illness, injury, or pregnancy. You must be diagnosed as totally disabled by a qualified physician, remain totally disabled during the 90-day elimination period, and meet all criteria for the plan. The benefit will be reduced by any income received from other sources, such as other disability programs, Social Security, or retirement on a dollar for dollar basis. This coverage is paid for entirely by Lee County.  The max salary range coverage is $100,000.00, with a maximum benefit of $1,154.00 per week.  This benefit is taxable. 

Short-Term Disability Service

This insurance will provide a benefit of 60% of your pre-disability base salary, reduced by any other deductible income you receive, up to a maximum salary of $1,000 per week, with a maximum benefit amount of $600 per week, and a minimum benefit amount of $50 per week. You will be responsible for using only 40 hours of your own accumulated sick leave. Your Short Term Disability benefits will begin on the 8th day of disability, and continue for a maximum of 12 weeks per occurrence. This coverage, if elected, is paid for entirely by the employee.  The max salary covered is $52,000.00 per year, with maximum benefit of $600.00 per week.  Benefit is not taxed.

           Medical Reimbursement Account (MRA)

The MRA allows you to pay for your medical, dental, or vision expenses that are not covered under your health plan with pre-tax dollars that are deducted from your bi-weekly pay. With this account, you can pay for your out-of-pocket medical expenses during the plan year for yourself, your spouse, and all dependents you list on your federal tax return, whether or not they are covered on any of the Lee County Benefits plans. The maximum you may place in this account for the plan year is $5,000. Remember to be conservative in your estimate - unused funds in your account will be forfeited!  There is a grace period of two and a half months after the end of the calendar year in which the deductions are taken from your paycheck pre-tax.  Expenses must be incurred and paid by March 15th of the following year.  You have until April 15th to request reimbursement.

Dependent Care Reimbursement

The Dependent Care Reimbursement Account works much like the Medical Reimbursement Account. You can contribute from $600 to $5,000 a year to a Dependent Care Account, before taxes are deducted from your pay, and taxes are never paid on these funds. When you have an eligible dependent care expense, you are reimbursed from your Dependent Care Account - tax free. Your contributions will be automatically deducted in equal amounts from each paycheck throughout the year. This account can be used for daycare expenses for the following dependents: Children under age 13, disabled children of any age, or your physically or mentally disabled spouse, dependent parents, or your spouse's dependent parents.  Again, you must pay expenses FIRST, then request reimbursement from your account. here is a grace period of two and a half months after the end of the calendar year in which the deductions are taken from your paycheck pre-tax.  Expenses must be incurred and paid by March 15th of the following year.  You have until April 15th to request reimbursement.

 Pretax Premium Deductions

The County offers a voluntary program that allows you to have your medical, dental, and vision premiums deducted from your gross pay before taxes are calculated.  Taxes are never paid on these premiums.

Vision Insurance; VSP Flyer

With this plan, members are eligible to receive coverage for routine eye exams and lenses every 12 months and frames every 24 months through the Vision Service Plan network of participating providers. The co-payment for an eye exam is $15. The co-payment for lenses or frames is $25, with only one $25 co-payment when lenses and frames are purchased together. This coverage, if elected, is paid for entirely by the employee.

Sick & Vacation Leave

A new, full or part-time (at least 20 hours per week) employee may accrue up to 13 days of sick leave and 12 days of vacation leave per year (depending on the number of scheduled hours of the position).

Holiday

A regular, full or part-time (at least 20 hours per week) employee may receive up to 11 paid holidays per year.

Bereavement

A regular, full or part-time (at least 20 hours per week) employee may be granted, upon request and with the approval of the Department Director, bereavement leave with pay due to a death in the family.

Civic Duty Leave

A regular, full or part-time (at least 20 hours per week) employee may be granted paid leave to serve on jury duty, vote in elections, and fulfill military duty.

Educational Assistance

It is the policy of Lee County Government to provide educational assistance to its employees to allow them to enhance their ability to serve the public interest of the citizens of Lee County.

Sick Leave Buy Back

Employees who have 200 hours or more of accumulated, unused sick leave may sell those unused sick leave hours accrued during the current fiscal year ONLY.  If you elect to sell your hours and meet the requirements, payment will be made to you sometime in October/November of each year.  This payment is subject to both income tax and social security deductions, and does not add to your final average compensation when your Florida Retirement System benefit is calculated.

Sick Leave Pool

The sick leave pool is a voluntary fund of donated sick leave, which has been established to provide limited additional sick leave benefits to participating members in the case of serious personal illness or injury.  Sick leave pool hours may not be used for the illness of a spouse or other family member.  You must have 80 hours of your own accumulated sick leave in order to join the pool.  You may join ONLY during open enrollment each year.  An annual donation of one day's pay is also required.

Direct Deposit

You may elect this convenient service directly through Payroll.  Your paycheck will be automatically deposited into an account in the bank of your choice.

Series E Savings Bond

U.S. Savings Bonds are available through payroll deduction.  Savings bonds are an easy, risk-free way to save.  You may sign up directly through Payroll.

Credit Union

As a Lee County employee, you are entitled to membership in Suncoast Schools Federal Credit Union (SSFCU).  You may become a "member" by opening a savings account in the amount of $5.  Call the SSFCU Member's Call Center at 813-621-7511 directly, or 800-999-5887 for more information.

Additional Information - Nationwide Retirement Solutions

Regular full-time employees scheduled to work at least 30 hours per week are eligible to participate in the Deferred Compensation Plan, the county's supplemental retirement program.  The County offers two plans in which you may participate:  Nationwide Retirement Solutions (formerly National Association of Counties); or, ICMA.  Contributions to these plans are totally provided by the employee through regular payroll deductions.

Florida Retirement System

All regular full-time employees (some part-time employees may also qualify) are automatically enrolled in the Florida Retirement System, and begin earning this benefit from the first day of their employment. You will be fully vested after six (6) years of employment.  Full retirement age after vesting is 62. Alternate Social Secruity

Is a pre-tax retirement savings program provided for those employees who are hired as "On-Call" or "Temporary."  Qualified employees contribute 7.5% of their pre-tax income into an account.  These contributions earn interest at the current declared market rate.  Enrollment is automatic with your employment.  All the money in your account - your contributions and any earned interest – is yours to withdraw when you terminate employment or retire.
