New Permit Application forms for Well Permitting that will be used statewide from the South Florida Water Management District. The new form does not address Soil Borings or Elevator Shafts; therefore, you will continue to use the Lee County forms for these two permits & Completion Reports. All other permit applications and completion reports must be completed using the SFWMD forms. The forms are available below.
