The Lee County Health Department's Immunization Programs for adults and children provide primary prevention of communicable diseases through immunizations for diphtheria, tetanus, whooping cough, polio, measles, mumps, rubella and Haemophilus influenzae-type B, hepatitis B and chickenpox.

 * Shots for flu, pneumonia and International Travel are also available *

FREE OF CHARGE up through the age of 18

Statutes

A Florida Certificate of Immunization (Department of Health Form 680) must be used to document that a child is fully immunized  This can be obtained from your physician or from any Florida Health Department.
Fully immunized includes the following:

five (5) doses of diphtheria, tetanus and pertussis *
four (4) doses of polio
two (2) doses of measles
one (1) dose of mumps vaccine
one (1) dose of rubella
three (3) doses of hepatitis B vaccine
three (3) doses of Rotavirus- all before age 16 weeks
Varivax (chickenpox) one (1) dose of vaccine OR history of disease needed for preK, Kindergarten, 1st & 3rd grades

* A tetanus-diphtheria booster may be necessary on entrance to seventh grade.

Requirements for compulsory immunizations for public and private preschools,  schools and childcare facility admittance and attendance are found under Florida Statutes (F.S.) 232.032 and 402.305, and Florida Administrative Code (F.A.C.) Rules 64D-3.011 and 65C-22.006.
