Hours are 9:00 AM to 4:00 PM Monday through Friday.
The following records are available from our office:
Florida Records:
Births: 1930 through present
Deaths: 2000 -- present
Fees for Birth Certifications:
Florida Records: $12. Addition copies of same @ $8.
Fees for Death Certifications:
Lee County Only: $10.
Requirements for Birth Certifications (please read completely)
- Issued only to the person named on the record when 18 years of age or older, or to the
registrant's parent or legal guardian;
- Legal guardian must show guardianship papers;
- Must have a signature ID;
- Payment in cash or money order. This office will not accept personal checks.

Requirements for Death Certifications:
Anyone may obtain a death certificate
without the cause of death.
A death certificate with the cause of death can be issued as provided in
section 382.025(2)(a) F.S., "To the registrant's spouse or parent, or to the
registrant's child, grandchild, or sibling, if of legal age, or to any
family member who provides a will, insurance policy or other document that
demonstrates his/her interest in the estate of the registrant, or to any
person who provides documentation that he/she is acting on behalf of any of
them..."
- Must be a family member of legal age or an attorney representing the family member;
- Payment in cash or money order. This office will not accept personal checks.

Birth Certification:
Send a notarized request with the information for the record requested, a legal-sized,
stamped, self-addressed envelope and a money order.
Death Certification:
Send a request with the vital information for the record requested, a legal-sized,
stamped, self-addressed envelope and a money order.
Death records before 1995 must be requested from the State Office in Jacksonville, Florida. Birth
records are $9 and death records are $5. Additional information can be requested for
the local office.
